Does integrated physical and mental condition care change outcomes in complex patients?

Regardless of whether integrated general medical and mental health services are sponsored by a care delivery system, a health plan through benefit descriptions, an employer through contract benefits, a government agency through public program features, or a care management company, when properly configured, integrated mental condition services in general and specialty medical settings can be expected to result in improved patient outcomes for targeted conditions. Data are no longer subtle. A recent meta-analysis of 37 randomized controlled trials integrating depression care with general medical care showed that incremental benefit occurs for least five years in depression. Value is documented by an increase in free days and reduced impairment (see Figure below).

Most of the data on integrated care relate to studies performed in diabetic, cardiac, or elderly patients with depression. These studies clearly show an increase in depression free days for those receiving an integrated care intervention. Associated benefits from integrated service delivery include improved adherence to clinical recommendations, reduced patient impairment, and increased patient and referring physician satisfaction with the mental health services provided (see Table and Figures below).
