the Case Management Society of America's ICM-CAG documentation software through training in complexity-based
integrated
case management principles, the methodology of INTERMED Case Management-Complexity Assessment
Grid (ICM-CAG) assessments, the use of ICM-CAG results in care plan development,
and the implementation of care plans designed to reverse barriers to improvement. ICM training
is designed to help care managers lower cost in chronically ill and complex patients
by improving their health. Measurement of clinical, functional, financial,
satisfaction, and quality of life outcomes is a core component of the ICM
process.
