Integrated Care Management

Adult Integrated Care/Case Management Training Course

Four types or ICM training are available:

  1. Full Integrated Care/Case Management Implementation Training
  2. Training for Care/Case Management Enrollment and Support Personnel
  3. Physician/Medical Director Training in effectively working with ICM managers and bringing value to patients/members
  4. Customized Cross-Disciplinary Training for Those Not Using CMSA-Based ICM Software or Workflows

The first three are available through the Case Management Society of America, provided in collaboration with Cartesian Solutions, Inc.™ The fourth is available only through Cartesian Solutions, Inc.™

Full Integrated Care/Case Management Implementation Training

The ICM certificate of completion course is an advanced practice case management program designed to give case and disease managers with nursing, social work, or other suitable health backgrounds the ability to perform interdisciplinary complexity assessments and to develop and implement care plans that address barriers to improvement in the biological, psychological, social, and health system domains. It is possible for early career case managers with an appreciation for basic case and/or disease management standards of practice to satisfactorily complete ICM training. Training includes more than 40 hours of nursing, case management, or social work continuing education credits. Those who study The Integrated Case Management Manual, participate in the 4 webinars, complete the face-to-face training, and pass the final examination will receive a certificate of completion and a username and password for clinical use of CMSA's privacy compliant ICM software.

Tuition

For members of the Case Management Society of America (CMSA), the webinar portion is $400 per person and face-to-face training is $250 per person. The tuition for non-members is $500 per person for the webinars and $350 per person for face-to-face training. All participants must have a copy of The Integrated Case Management Manual: Assisting Complex Patients Regain Physical and Mental Health to study as a text for the course. (Twenty percent discounts for Manual purchase are available through CMSA and Cartesian Solutions, Inc.™) Webinar training is a prerequisite for enrollment in the face-to-face training.

Training for care/case management enrollment and support personnel

ICM enrollment and support specialists do not require full training in ICM procedures to perform care/case management enrollment and support tasks on behalf of patients/clients and the ICM case managers working with them. They, on the other hand, must be aware of how the multi-domain approach to complexity assessment works, know about the negative impact of untreated mental conditions on medical and cost outcomes, understand the principles of motivational interviewing, and be familiar with connection of the complexity assessment to care plan implementation and goal achievement. Enrollment and support specialists typically do not work with patients/clients/members but rather assist the care/case managers who do in completing important preparatory and follow-up assignments that lead to patient/client health, reduced health care service use, and return to maximum personal function. Those participating in enrollment and support personnel training will read The Integrated Case Management Manual and login to four two-hour webinars adapted to cover enrollment and support roles. They will receive continuing education credits for their participation.

Physician Director Training

Non-psychiatrist and psychiatrist physicians, working in the roles of a health plan Medical Director or the director of a clinic or inpatient service, are key personnel to have involved as ICM care/case managers perform their management activities. Unfortunately, even physician medical and clinic directors have a limited understanding of the power that ICM has in improving outcomes and reducing costs for their patients. Some physician directors are also uncertain who they should be referring to their care/case management colleagues and how active physician case review, collaborative target outcome definition, and escalation of care in patients failing to improve can substantially contribute to care/case manager effectiveness.

Case managers do not "treat" patients. Rather, they are trained to "assist" clinicians and patients overcome barriers that prevent from returning to health. Most do not have the clinical expertise to know next steps to take when patients are not improving. This is where physician directors and the clinicians that they supervise add value to patients' care/case management experience. Interdisciplinary physician directors provide the clinical expertise necessary to review and facilitate "next steps" in care for complicated patients stymied in their road to stabilized health. The case/care manager and physician director team, using an outcome orientation (treat to target), support and assist patient's clinicians directly and indirectly as they escalate treatment to approaches with greater potential to recapture health.

Cartesian Solutions, Inc.™ physician director training reviews the literature related to core principles of care/case manager physician collaboration that bring value to complex patients; the importance of the medical/mental condition interaction, especially in chronic and multi-morbid medical and mental health patients; and the augmentation in outcomes that "supervising" or "collaborating" physicians/medical directors can bring to outcome enhancement in the care management environment.

Like other trainees in ICM, physician directors would benefit by reading The Integrated Case Management Manual: Assisting Complex Patients Regain Physical and Mental Health in addition to participating in four ICM webinars modified specifically for physician directors. The modified webinars cover the principles of "treat to target," how to work with and assist treating clinicians as they escalation care in complex patients who are not improving. The webinars allow physician directors to understand the work processes of ICM managers and to better appreciate non-biological and psychological factors that create barriers to improvement. At the completion of webinar training, physicians will be able to maximize health in complex patients, reduce impairment, and lower total health care service use.

Webinar training is $750 per physician directors and/or clinicians working with care/case managers. All participants are encouraged to buy a copy of The Integrated Case Management Manual: Assisting Complex Patients Regain Physical and Mental Health and to read it in preparation for the webinar course. Payment

Interested case or disease managers for CMSA courses should obtain Participant Applications available on the CMSA website. Participant applications should be completed and returned to CMSA for application processing prior to course closure. Alternatively, a case and disease management program affiliated with a hospital and clinic system, a health plan, a government program, etc. can obtain a Site Application if there is a desire to have course offerings performed for groups of 21 participants or more. If a program does not have 21 or more trainees but wishes onsite training, Cartesian Solutions, Inc.™ or CMSA can provide a pricing estimate for local face-to-face training upon request. Site Applicants can choose to do the face-to-face component at a location of their choosing or at one of the regional locations sponsored biannually through CMSA and Cartesian Solutions, Inc.™ It should be noted that courses close several weeks before the onset of webinars to give trainees time to complete preparatory reading and study.

Payment for courses by purchasers with multiple participants from one organization (Site Applications) must be made in one check from an organization to receive the multi-participant discounts shown above. This payment must be accompanied by a list of participants taking the course. The fee schedule is subject to change without notice.

Customized Cross-Disciplinary Training for Those Not Using CMSA-Based ICM Software or Workflows

A number of organizations or companies wish to incorporate the value-enhancing complexity-based approach to integrated medical and mental health case management but do not wish to or cannot change the general work processes or software that care/case managers use within their system. To meet needs for these potential clients, Cartesian Solutions, Inc.™ creates customized programs that adapt the introduction of medical or mental health care/case management to the service location. It covers the basics of complexity-based interdisciplinary management procedures. At the completion of training, care/case managers working at health plans or within a care delivery systems will understand the principles of integrated case management, the importance of a multi-domain approach to care/case management and of the medical/mental health interaction, how to assist patients/clients with cross-disciplinary illnesses while avoiding handoffs, and work processes that can lead to clinical, functional, and cost outcome improvement.

Customized training does not include training in anchoring barriers to improvement in the Integrated Case Management-Complexity Assessment Grid (ICM-CAG) nor in the use of its associated CMSA software. It does, however, utilized the ICM-CAG as a teaching aid since the complexity, rather than disease, approach to care/case management expands the reach and capabilities of care/case managers and their organizations that wish to maximize health improvement and cost reduction through integrated medical and mental health care/case management. Those participating in customized training will be expected to read The Integrated Case Management Manual and participate in four two-hour webinars adapted to managers coming from medical and mental health backgrounds that wish to address the biopsychosocial and health system needs of their patients/clients. For companies wishing to implement integrated care/case management practices into their local, regional, or national system, customized onsite face-to-face training can be arranged.

Physician directors working with care/case managers within a customized systems are encouraged to take the physician director training described above. It is applicable to both customized and full ICM programs.

Tuition

Basic customized webinar training is $500 per person. All participants are required to have a copy of The Integrated Case Management Manual: Assisting Complex Patients Regain Physical and Mental Health for study. (Twenty percent discounts for Manual purchase are available through CMSA and Cartesian Solutions, Inc.™) Face-to-face training costs will depend on the number of trainees, existing care/case management methods in use, and desired outcomes for participating care/case managers. Clients are encouraged to contact Cartesian Solutions, Inc.™ to describe desired deliverables and to request fees associated with an onsite consultation.
