Yasuhiro Kishi, M.D.
Integrated Care Organization and Delivery Specialist
Dr. Kishi currently serves as Associate Professor in Psychiatry
and is Director of the Psychiatric Service at Nippon Medical School,
Musashikosugi Hospital, Kawasaki Japan. He has extensive experience
in intercultural clinical, health system, and research activities
dealing with medical and psychiatric illness. He serves as the Cartesian
Solutions, Inc.™ Japan and Asian countries consultation representative
supplying comparable capabilities for those wishing to set up economically
advantageous integrated programs among countries in the Far East.
Following Dr. Kishi's residency and fellowship in General Hospital
Psychiatry in Japan, he worked as a fellow on the Complexity Intervention
Unit at the University of Iowa for two years under the mentorship
of Dr. Kathol. During this time he became intimately familiar with
the interaction of general medical and psychiatric illness, its treatment,
and the clinical organizational components of complexity intervention
units and value-added mental condition services that lead to outcome
changing care in patients with health complexity. Armed with training
in General Hospital Psychiatry and the organization of integrated
physical and mental health services, he returned to Japan and worked
as a general hospital psychiatrist.
When Dr. Kathol took a position with Blue Cross Blue Shields Minnesota
in 2001, Dr. Kishi returned to the U.S. as a psychiatric fellow at
the University of Minnesota to hone his understanding of health systems
and how they influence the organization and delivery of coordinated
physical and mental health services. It was during this two-year
period that Dr. Kishi developed a comprehensive appreciation for
the connection between integrated clinical service capabilities and
the organization of the health system in which services are delivered.
Since he has had extensive experience in the delivery of care in
Japan and the U.S. as well as collaborative work with academic psychiatrists
and non-psychiatrists in many other countries throughout the world,
Dr. Kishi is well positioned to understand the components of care
and the health system features that lead to patient health and efficient
use of health resources through the integration of physical and mental
condition services, particularly in patients with health complexity.
Dr. Kishi works with colleagues in the Society of General Hospital
Psychiatry in keeping the Japanese government informed regarding
the effect of health care containment policies, which could segregate
physical from mental health care. One of their recent successes was
to advocate for inclusion of psychiatric care in newly developed
Japanese diagnostic related groups (DRGs). This initiative prevented
isolation of mental health care from general and specialty medical
care delivered to patients with concurrent behavioral and physical
health disorders, which are commonly seen in complex and high cost
patients. Dr Kishi is actively involved in other clinical and system
issues which alter access to and outcomes for complex patients with
combined medical and psychiatric illness, both in Japan and with
international colleagues. Much of his current research is in cross-cultural
health policy and health systems, but always as it relates to improving
clinical care.
Dr. Kishi completed his medical training at Nippon Medical School.
He has published over 120 scientific articles and 30 book chapters
in Japanese and English. Dr. Kishi serves as a member of the Executive
Committee of the Japanese Society of General Hospital Psychiatry.
